Full-time is defined as undergraduates enrolling in at least 12 semester hours, or equivalent, and graduate students enrolling in at least 9 semester hours, or equivalent.<\/p>\n\n\n\n<p><strong>Note:&nbsp;<\/strong>A hold is automatically set to prevent a student&#8217;s registration until the process is completed. Once completed, the hold is automatically removed and the student is clear to register.<em>&nbsp;<strong>There is no need to contact Health Services to remove the hold since everything is automatic.<\/strong><\/em><\/p>\n\n\n\n<div class=\"wp-block-buttons is-content-justification-center is-layout-flex wp-container-core-buttons-is-layout-a89b3969 wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link has-mtsublue-background-color has-background wp-element-button\" href=\"https:\/\/www.mtsu.edu\/healthservices\/certificate-of-immunizations\" style=\"border-radius:5px\" target=\"_blank\" rel=\"noreferrer noopener\">Certification of Immunizations Form<\/a><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Directions for Submitting Your Immunizations &#8211; Registration Compliance<\/h2>\n\n\n\n<p>In order to register for classes Health Services must have proof of&nbsp;<strong>two doses of Varicella<\/strong>&nbsp;(chicken pox) and<strong>&nbsp;two doses of Measles, Mumps &amp; Rubella<\/strong>&nbsp;(MMR). If you&#8217;re under the age of 18, you must complete the additional form found in the student portal. Students living on campus must have proof of Meningitis strains A, C, Y &amp; W135 (either Menactra\u00ae or Menveo\u00ae).<\/p>\n\n\n\n<p><em><strong>If we suspect that the Certification of Immunization form has been falsified, we will require one of the following: official copies of the student&#8217;s immunization record or a blood titer test to prove immunity.<\/strong><\/em><\/p>\n\n\n\n<ul class=\"wp-block-list is-style-compact-list\">\n<li><strong>STEP 1<\/strong>:\n<ul class=\"wp-block-list\">\n<li>Log into the&nbsp;<a href=\"https:\/\/mthealth.mtsu.edu\/confirm.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">student portal<\/a>.<\/li>\n\n\n\n<li>Click on\u00a0<strong>Medical Clearances<\/strong>\u00a0on the left column.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>STEP 2<\/strong>:\n<ul class=\"wp-block-list\">\n<li>Click <strong>Add Immunization Record<\/strong> to upload your immunization documentation.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>STEP 3<\/strong>:\n<ul class=\"wp-block-list\">\n<li>Scroll down the page to view the <strong>items required for clearance<\/strong>.<\/li>\n\n\n\n<li>The <strong>Status <\/strong>column will show either Compliant or Not Compliant.<\/li>\n\n\n\n<li>For <strong>non compliance<\/strong> you must add each date of the immunization that is listed as Not Compliant.\n<ul class=\"wp-block-list\">\n<li>Click on the red Update button.<\/li>\n\n\n\n<li>Click Add Immunization.<\/li>\n\n\n\n<li>Add the date your immunization was administered.<\/li>\n\n\n\n<li>Select the immunization from the dropdown box.<\/li>\n\n\n\n<li>Click Save.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Please allow 48-72 hours for processing.<\/strong><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Immunization Information<\/h2>\n\n\n\n<div class=\"wp-block-mtsu-blocks-expandable-block accordion expandable-group expandable-group-lightblock\" id=\"newExpandable\">\n<div class=\"wp-block-mtsu-blocks-expandable-item accordion-item expandable-heading expandable-heading-lightblock\"><h2 class=\"accordion-header\" id=\"headingimmunization-requirements-amp-recommendations\"><button class=\"accordion-button collapsed\" type=\"button\" data-bs-toggle=\"collapse\" data-bs-target=\"#collapseimmunization-requirements-amp-recommendations\" aria-expanded=\"false\" aria-controls=\"collapseimmunization-requirements-amp-recommendations\">Immunization Requirements &amp; Recommendations<\/button><\/h2><div id=\"collapseimmunization-requirements-amp-recommendations\" class=\"accordion-collapse collapse expandable-content expandable-content-lightblock\" aria-labelledby=\"headingimmunization-requirements-amp-recommendations\" data-bs-paren=\"#newExpandable\"><div class=\"accordion-body\">\n\n<ul class=\"is-style-compact-list wp-block-list\">\n\n<li>MMR:&nbsp;All students born on or after Jan. 1, 1957 must provide proof of immunization with two doses of MMR vaccine at least 28 days apart or serology showing immunity to MMR.<\/li>\n\n\n<li>Varicella or Chickenpox:&nbsp;All students born on or after Jan. 1, 1980 must provide proof of immunization with two doses of Varicella vaccine at least 28 days apart, serology showing immunity to Varicella or documentation from a medical facility verifying a previous diagnosis with the illness<\/li>\n\n\n<li>Meningitis:&nbsp;New incoming students who are younger than 22 years of age and who will live in campus housing must have documentation of a dose of quadrivalent conjugate vaccine (MCV4 protects against strains: A, C, Y, W135) at greater than or equal to 16 years of age. All new students will need to electronically sign an acknowledgment form before they can register. You should check with family members who may have copies of childhood records. You should try to contact the original immunization provider: if a local health department, contact them directly; if a private medical office, contact that office. Schools may have copies of immunization certificates in student files. Children born after the mid-1990&#8217;s may have records entered in a state-managed immunization registry; such registries now exist in many states, but are unlikely to contain information on adults. If records cannot be located, additional vaccinations are recommended.<\/p>\n\n\n<p>If records cannot be located, you can have a blood titer test completed. If the test verifies immunity, this will satisfy the requirement. This test can be completed in our office, another provider\u2019s office, or your local health department.&nbsp; In our office, the MMR titer is $40.00 and the Varicella is $20.00.&nbsp; Prices subject to change.<\/p>\n\n<\/div><\/div><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Students Taking Online Classes Only<\/h2>\n\n\n\n<p>If you are a&nbsp;<strong>full-time online only<\/strong>&nbsp;student,&nbsp;<a href=\"https:\/\/itdwebapps.mtsu.edu\/DFPassThrough\/DFCasAuth?TargetResource=https:\/\/dynamicforms.ngwebsolutions.com\/ShowForm.aspx?RequestedDynamicFormTemplate=be9595f6-53fd-4976-9dc3-5b19b8b233dd\" target=\"_blank\" rel=\"noreferrer noopener\">click here<\/a>&nbsp;to be taken to the online exemption form.
