If you answer YES to any of the questions in Part I of the first page, you MUST complete page 2.<\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><a href=\"https:\/\/www.mtsu.edu\/healthservices\/certificate-of-immunizations\" target=\"_blank\" rel=\"noreferrer noopener\">Meningitis&nbsp;MCV4 \u2013 Serogroups A,C,W-135, &amp; Y<\/a>\n<ul class=\"wp-block-list\">\n<li><strong>Only for students living on campus<\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><a href=\"https:\/\/www.mtsu.edu\/healthservices\/certificate-of-immunizations\" target=\"_blank\" rel=\"noreferrer noopener\">Varicella (Chicken Pox)<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/www.mtsu.edu\/healthservices\/certificate-of-immunizations\" target=\"_blank\" rel=\"noreferrer noopener\">MMR (Measles, Mumps and Rubella)<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/forms.office.com\/Pages\/ResponsePage.aspx?id=QL8udrKAukCG_m3UCay0mTjbllxs06JPj_hJPGzivLpUNFJLNkFJOE9YT0dQQU81NU42UFpDQzRWNiQlQCN0PWcu\" target=\"_blank\" rel=\"noreferrer noopener\">Hepatisis B\/Meningitis<\/a>\n<ul class=\"wp-block-list\">\n<li><strong>If student is under the age of 18, this must be completed by a parent.<\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Directions for Submitting Your Immunizations &#8211; Registration Compliance<\/h2>\n\n\n\n<p>In order to register for classes Health Services must have proof of two doses of Varicella (chicken pox) and two doses of Measles, Mumps &amp; Rubella (MMR). If you&#8217;re under the age of 18, you must complete the additional form found in the student portal. Students living on campus must have proof of Meningitis strains A, C, Y &amp; W135 (either Menactra\u00ae or Menveo\u00ae).<\/p>\n\n\n\n<p><em><strong>If we suspect that the Certification of Immunization form has been falsified, we will require one of the following: official copies of the student&#8217;s immunization record or a blood titer test to prove immunity.<\/strong><\/em><\/p>\n\n\n\n<ul class=\"is-style-compact-list wp-block-list\">\n<li>Log into the&nbsp;<a href=\"https:\/\/mthealth.mtsu.edu\/confirm.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">student portal<\/a>.<\/li>\n\n\n\n<li>Click on&nbsp;<strong>Medical Clearances<\/strong>&nbsp;on the left column.<\/li>\n\n\n\n<li>The Status column will show either Compliant or Not Compliant.\n<ul class=\"wp-block-list\">\n<li>Click the&nbsp;<strong>Update&nbsp;<\/strong>button if your status is Not Compliant and follow the directions.<\/li>\n\n\n\n<li>Please allow 48-72 hours for processing.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Required Immunizations All international students MUST complete the following forms before they can register for classes.
