Forms for Off-Campus Users
MTSU complies with the Federal Family Educational Rights and Privacy Act. Information that is not directory information will not be released.听Current directory information at MTSU is defined as name, city of permanent place of residence,聽聽photograph or video when related to a MTSU activity, year of birth, major field of study, enrolled status (full-time, part-time, withdrawn), student level (freshman, sophomore, etc.), dates of attendance, degree status (expected graduation date and/or dates conferred),聽University-recognized honors and awards and dates received, participation in officially recognized activities and sports, height and weight of athletes, and the most recent educational agency or institution attended by the student.听
Records will be produced electronically in a PDF format whenever feasible as a means of utilizing the most cost effective method of producing records.听 MTSU Policy 120 Public Records – Inspecting and Copying may be found聽here.听
